DuPage Dietitians understands your need for high quality care at affordable prices. That's why we have made unprecedented efforts to lobby insurance companies to cover your nutritional counseling and will continue to negotiate with other major health care insurers to afford you the best in nutrition counseling.

We provide "in-network" services for the following insurance companies:

• Humana
• United Healthcare
• Aetna
• Great West Health Care (now a part of Cigna)
• Union Health Services

We are happy to guide you in determining how your insurance coverage may help defray all or some of the costs of nutrition therapy.

Here is a list of things you will need to do:

1. Call your insurance company and confirm that you have a nutrition counseling benefit. Because DuPage Dietitians is in-network, that does not necessarily mean your plan covers nutrition therapy. Inquire specifically about CPT codes 97802 and 97803. Ask for written confirmation in the form of a pre-authorization.

2. Obtain a diagnosis from your physician. A diagnosis provided by your doctor is required for us to submit claims on your behalf. This is not the same thing as a referral. Please bring this form to your first visit: Diagnosis form

3. If you do not have a specific medical problem, you may still be able to have your visits covered under a "V" code. Inquire about this as well. Additionally, your employer may have a wellness benefit you can utilize, or you might have an FSA or HSA you can utilize.
